Medicaid Home & Community Based Waiver
This is a state-wide long term care program designed to help frail elderly and disabled persons avoid inappropriate nursing home placement by providing supportive services.

- Personal Care/ Supervision
- (PERS) Personal Emergency Response System
- Private Duty Nursing
- Training Services
- Transportation
- Environmental Modifications
- Specialized Medical Equipment and Supplies
- Nursing Facility Transition Services
Age/Financial Eligibility
- 18 years and older and disabled or 65 or older
- Income no greater than $1911 a month, gross
- Assets no greater than $2000 for single person
- Special rules for couples
Medical Criteria
- Frail elderly or disabled
- Eligible for nursing home placement
- Need for at least one Covered Service
The Area Agency on Aging's waiver staff can take your calls and an assessment in the home, hospital, or nursing home can be scheduled with one of our care coordination teams.
